VCSE Health & Wellbeing Fund, Social Prescribing, deadline 21 November 2017

The Department of Health and Public Health England are inviting applications to the Health and Wellbeing Fund to set up a new social prescribing scheme or to support the expansion of an existing scheme.

How much and what for?

Up to £300,000 is available (tapering over up to 3 years).

The funding for 2017/18 is for activities that support social prescribing which is defined as 'enabling healthcare professionals to refer patients to a link worker, to co-design a nonclinical social prescription to improve their health and wellbeing'.

The funder is seeking applications with design models that fit the needs of their local area and draw upon local assets.

Applications for both new and existing social prescribing schemes will be considered. 

Assessment criteria

In your responses to the questions in the application form, you will need to provide details of:

  • Commitment from local partners for resourcing
  • The model that your scheme will use
  • For existing schemes, how the grant funding will support expansion
  • Evidence of working with local partners
  • How the scheme will reduce health inequalities and how it will support access for protected
  • characteristic groups
  • How you intend to measure the impact of your scheme.
